Please upload a copy of Your Organization's IRS EIN Number (Employer Identification Number)

Submit a copy of ONE of the following documents listed below with your Nonprofit Marketing Grant Application:

* Corporate Secretary of State Filed - Articles of Incorporation

* IRS Internal Revenue Services Filed - 501(c)3 Determination Letter OR

* FTB Franchise Tax Board Determination Letter OR

* Fictitious Business Statement (DBA) - County Recorder Filing
